Flashcards in Allman - Fleuro, Tetra Deck (18):
1

Quinolone mechanism of action

DNA Gyrase -- messes up supercoiling of DNA
- cidal

2

Quinolones and Absorption

Food decreases rate but not extent of absorption
**ANACTIDS / CATIONS decrease the oral bioavailability!

3

Quinolone Drug Interactions

1. Theophylline: (decreases metabolism) Cipro = DOUBLES the levels, Levo = no effect

2. Antacids/Iron/Sulfate: interferes with Q absorption if given within 2 hours of each other

3. Warfarin: Increases anticoag effects; Levo - no changes;

4

Quinolone Adverse Effects

Photosensitivity
CNS: Headache, Dizzy
- Mental Status Change (elderly)
MSK: Arthropathy, Tendon Rupture
Heart: Longer QT

5

Quinolone Spectrum of Activity

Cipro: EXCELLENT Gram - coverage
Levo, Gemi, Moxi: Gram + (staph and strep) and Gram -

6

Quinolone Uses

UTI - NOT moxi,gemi
Chlamydia
GI
Skin - Cipro, Oflox
Bone and Joint - Cipro, Oflox
Intra-abdominal Infections - Cipro, Moxi

7

Ciprofloxacin Facts

- Most potent quinolone for gram -
- 4x more potent for PSEUDOMONAS
- Oral form has excellent bioavailability

8

Levofloxacin/Gemifloxacin/Moxifloxacin Facts

- Gram + coverage
- Gram - (not as potent as cipro)
- Atypicals

9

What is special about Moxifloxacin?

It covers intra-abdominal infections (anaerobes) but NOT UTIs

10

Tetracycline Mechanism of Action

Reversibly binds 30S
- static
- absorption decreased by food
- different bioavailability depending on agent (doxy high, tetra low)

11

Elimination of Tetracyclines

Renal = Tetra, Oxytetra, Deme
Hepatobiliary = Doxy, Mino

12

Tetracycline adverse Reactions

Photosensitivity
Discolored Teeth (kids under 8)
Mino -- dizzy, GOOD FOR ACNE

13

Faconi-Like-Syndrome

Happens because of an outdated Tetracycline
- N/V, lethargy, polydipsia, polyuria, proteinuria, acidosis, hypokalemia

14

Tetracycline drug interactions

Di/Tri valent cations
Antacids
Dairy
Warfarin

15

Tetracyline Coverage

Staph, Strep
Piddly -
Atypicals
Rickettsia (DRUG OF CHOICE)
ACNE
Vibrio (water)
Lyme Disease

16

What drug can you NOT give to children under 8 years old OR pregnant women?

Tetracyclines

17

What do you use to treat Brucellosis

Tetracycline + Gentamicin

18

What treats SIADH?

Demeclocycline
