What is it?

A

Upper airway obstruction due to inflammation. Caused by a VIRAL INFECTION (usually parainfluenza 1 or 3)

A

6 months - 6 years

Usual duration of illness

A

3-5 days

Presentation

A

Sudden-onset seal-like barking cough

Stridor, hoarse voice, resp distress

Coryza + fever may be present in first 24-48 hours

2 exacerbating factors

A

Worse at night + if agitated

Features of mild croup

A

Seal bark cough

NO stridor or intercostal recession

Features of moderate croup

A

Cough + stridor + sternal recession

NO agitation or lethargy

Features of severe croup

A

Inclused agitation + lethargy

Features of impending resp failure

A

Inc obstruction, interostal recession, asynchronous chest wall + abdo movement
Fatigue/ pallor/cyanosis

Mx of mild croup

A

Single dose of oral dexamethosone (0.15mg/kg)

A

ADMIT!!
Corticosteroids
NEBULISED EPINEPHRINE WITH 8L/MIN OXYGEN

DDx:

What is bacterial tracheitis?

A

Rare complication of flu

Fever, suden-onset stridor + resp distress following a resp illlness

May seem to be improving, then gets suddenly worse

DDx:

Retropharyngeal/ peritonsillar abscess

A

More gradual onset

Fever, unilateral cervical lymphadenopathy, neck stiffness
