Embolism

Detached intravascular solid, liquid, or gaseous mass that is carried by the blood to a site DISTANT from it's point of origin

Pulmonary Thromboembolism

Source = DVT from lower extremity
Often multiple (shower)
Most are silent clinically

Sudden death, right heart failure, or heart collapse with > 60% if pulm circ is obstructed

Obstruction of medium sized arteries = hemorrhage but not infarct bc of dual circ in lung

INFARCT

Systemic Thromboembolism

Travels in arterial circulation
80% from HEART
Some from paradoxical embolism (from right side -- bypasses pulmonary circ -- inter ventricular septal defect (R to L SHUNT)

Lodge in lower extremeties MOST or brain

Fat Embolism

From bone marrow after fracture of long bones

Fat Embolism Syndrome

1-3 days post injury
Pulmonary insufficiency
NEURO - irritable, restlessness, delirium, coma
Anemia, Thrombocytopenia

Obstructs pulm and cerebral microvasculature

Air Embolism

>100 cc
Decompression sickness

Amniotic Fluid Embolism

Infusion of amniotic fluid or fetal tissue into material circulation postpartum

Rare. Mortality is 20-40%
