Synthetic catecholamine, selective for beta adrenergic receptors

What are the primary uses of Isoproterenol?

A
  • chronotropic agent for heart transplant
  • “chemical pacemaker” for heart block and bradydysrhythmias
  • mild pulmonary vasodilating effects (reduce PulmVR)

What is the MOA of Isoproterenol?

A

Provides non-selective beta-adrenergic stimulation of Beta 1 > Beta 2
-also a pulmonary and systemic vasodilator
-activates beta adrenergic receptors via G protein coupled receptors.
Increases HR and contractility, with decreased SVR (Increased SBP; decreased DBP and MAP)

How is Isoproterenol metabolized and eliminated?

A
  • rapidly metabolized in the liver by COMT (need infusion!)

- 40-50% eliminated unchanged in the kidneys

A

1-5mcg/min (infusion 0.5-10mcg/min)

A
  • tachycardia
  • dysrhythmias
  • decreased coronary BF 2dary to decreased DBP
  • increased oxygen consumption d/t inotrope and chronotrope effects
  • peripheral vasodilation and hypotension

What are the contraindications to Isoproterenol?

A
  • Hypersensitivity
  • Tachycardia caused by Dig toxicity
  • CAD and angina

What are the drug interactions with Isoproterenol?

A

increased potential for arrhythmias with some general anesthetics and co-administration of other sympathomimetics
