What are the causes of megaloblastic anemia?

A

Folate or B12 deficiency or orotic aciduria due to UMPS deficiency.

How can you differentiate the orotic aciduria of UMPS deficiency from that of OTC deficiency?

A

Decreased BUN in OTC deficiency due to impaired urea cycle. Also no megaloblastic anemia in OTC deficiency.

How would you differentiate folate from B12 deficiency CLINICALLY?

A

Peripheral neuropathy in B12 deficient pts. (d/t role of B12 in fatty acid pathways.)

How would lab tests differ in folate- and B12-deficiency?

A

Methylmalonic acid increased in B12 deficiency.
