Antiphospholipid Antibody Syndrome

Abs bind to complexes "unveiled" by phospholipids
RECURRENT MISCARRIAGES, thrombocytopenia
2 types of pts: with autoimmune disease (SLE) - secondary or without - primary

Lines of Zahn

Thrombosis has laminations (FORMED IN FLOWING BLOOD)
Pale layers = platelets
Darker layers = fibrin and RBCs

Arterial Thrombi

Usually occlusive
Gray-White
Grows RETROGRADE to blood flow (toward HEART)

Venous Thrombi

Always occlusive
Lower extremities (90%)
Red (more RBCs)
Grow IN DIRECTION of blood flow (toward HEART)
Prone to embolization (LUNGS)

Post-Mortem Clots

Not attached to wall
dark red
yellow chicken fat supernatant

Superficial Venous Thrombosis

Rarely embolize
Cause edema DISTAL to obstruction

DVT

At or above knee may embolus
Sx of pain and edema relieved by collateral circa
Ultrasound of Angiogram
Treat - anticoag
BELOW KNEE: monitor 1-2 wks for propogation above knee
