_____: The PATIENT must ensure their FINANCIAL OBLIGATIONS for HEALTHCARE SERVICES are FULFILLED AS PROMPTLY AS POSSIBLE and make arrangements for any UNPAID BILLS.
_____: The PATIENT is OBLIGATED TO RESPECT THE RIGHTS and WELL-BEING of HEALTHCARE PROVIDERS, INSTITUTIONS, AND OTHER PATIENTS. They must FOLLOW the institution’s POLICIES, RULES, AND REGULATIONS.
_____: The PATIENT should REFRAIN from UNHEALTHY FOOD, ADDICTIVE SUBSTANCES like TOBACCO AND ALCOHOL, and RECKLESS LIFESTYLES. They are OBLIGATED TO MAINTAIN A STATE OF WELLNESS.

a.3 Patient’s Obligations
_____: The PATIENT must have ADEQUATE HEALTH INFORMATION to actively PARTICIPATE in their DIAGNOSTIC and TREATMENT PLANS. When they sign an INFORMED CONSENT, it is assumed they have the NECESSARY INFORMATION.
_____: The PATIENT must SUBMIT GRIEVANCES to the PROPER AUTHORITIES and not resort to UNWARRANTED PUBLICITY. They should not DISCLOSE any alleged COMPLAINT against a HEALTHCARE PROVIDER or INSTITUTION to the PUBLIC if it has not been fully decided by a COURT.
_____: The PATIENT is OBLIGATED not to BREACH PRIVILEGED COMMUNICATION, especially if it involves a THIRD PARTY.
_____: While a PATIENT can CHOOSE their PHYSICIAN, they are also OBLIGATED to RESPECT a PHYSICIAN’S DECISION to REFUSE TO TREAT them.

a.3 Patient’s Obligations
_____: A PATIENT is OBLIGATED to RESPECT a PHYSICIAN’S RELIGIOUS BELIEFS when it comes to MEDICAL DECISIONS. If the PATIENT is a MINOR or LEGALLY INCAPACITATED, their LEGAL REPRESENTATIVES are also OBLIGATED to RESPECT the PHYSICIAN’S DECISION, even if it CONFLICTS with their own RELIGIOUS BELIEFS.
_____: The PATIENT is OBLIGATED to ENSURE THE INTEGRITY AND AUTHENTICITY of their MEDICAL RECORDS. ALTERING these records is a CRIMINAL OFFENSE.
_____: The PATIENT is OBLIGATED to PARTICIPATE in the TRAINING OF FUTURE PHYSICIANS, provided they are given the NECESSARY INFORMATION and ETHICAL CONSIDERATIONS are followed.

_____: Every PERSON has a RIGHT to a CONTINUITY OF GOOD QUALITY HEALTH CARE without DISCRIMINATION and within the limits of the RESOURCES, MANPOWER, AND COMPETENCE available for HEALTH AND MEDICAL CARE.
_____: PATIENT’S DIGNITY, CULTURE AND VALUE shall be RESPECTED AT ALL TIMES in MEDICAL CARE AND TEACHING.
_____: Every PERSON has the RIGHT TO BE INFORMED of his RIGHTS AND OBLIGATIONS as a PATIENT.
_____: The PATIENT is FREE TO CHOOSE the services of a PHYSICIAN OR HEALTH INSTITUTION of his choice except when he chooses to be confined in a CHARITY WARD.
_____: The PATIENT has a RIGHT TO SELF-DETERMINATION and to MAKE FREE DECISIONS regarding HIMSELF/HERSELF, however, the ATTENDING PHYSICIAN shall INFORM the PATIENT of the CONSEQUENCES of his/her DECISIONS.

_____: The PATIENT has the RIGHT TO REFUSE DIAGNOSTIC AND MEDICAL TREATMENT PROCEDURES, provided that the following conditions are satisfied:
i. The PATIENT is of LEGAL AGE and is MENTALLY COMPETENT;
ii. The PATIENT is INFORMED of the medical CONSEQUENCES OF HIS/HER REFUSAL;
iii. The PATIENT RELEASES those involved in his care from any OBLIGATION relative to the CONSEQUENCES of his/her DECISION; and
iv. The PATIENT’S REFUSAL WILL NOT JEOPARDIZE PUBLIC HEALTH AND SAFETY.
_____: The PATIENT has the RIGHT TO BE ADVISED of PLANS TO INVOLVE HIM/HER IN MEDICAL RESEARCH that may AFFECT the CARE OR TREATMENT of his/her CONDITION.
