What does the SOKRATES acronym stand for ?

A

S - Site: Where is the pain located?
O - Onset: When did it start, and was it sudden or gradual?
C - Character: What does the pain feel like (e.g., sharp, dull, aching)?
R - Radiation: Does the pain move or spread to other areas?
A - Associations: Are there any other symptoms present (e.g., nausea, vomiting)?
T - Time course: Does the pain follow a pattern (e.g., constant, comes and goes)?
E - Exacerbating/Relieving factors: What makes the pain better or worse?
S - Severity: How bad is the pain on a scale of 0-10?
