A

Autoimmune disorder that attacks the thyroid gland.

Life threatening emergency by poor circulation, hypothermia, hypothyroidism. Treatment is IV levothyroxine.

Interferons, Tyrosine Kinase inhibitors, Amiodarone, Lithium, Carbamazepine. Acronym is I TALC.

A

Low free thyroxine is 0.9 -2.3. Normal TSH is 0.3-3.

Synthroid, Levoxyl, Unithroid. Full replacement dose is 1.6 mcg/kg/day (IBW). Therapeutic equivalence is orange book. If known CAD start with 12.5-25 mct daily,

Lower dose in cardiovascular disease. TSH levels check every 4-6 weeks until levels are normal. Levothyroxine is drug of choice. IV to PO is 0.75:1

A

Thyroid, Dessicated USP (T3 and T4).

Cytomel. Shorter half life.

Aluminum(antacids), calcium, cholestyramine, iron, magnesium, multivitamins, sevelamer and sucralfate. Beta blockers.

A

Graves disease can, iodine, amiodarone, and interferons, exposure to radiographic contrast dye.

Inhibit synthesis of thyroid. PTU is peripheral conversion. PTU can cause severe liver injury and acute liver failure with PTU. Pregnancy, PTU preferred in 1st trimester, methimazole often recommend for 2nd and 3rd trimesters. GI upset, DILE, hepatitis, agranulocytosis(rare).

A

temporarily inhibit secretion of thyroid.

Potassium iodide (KI) blocks the accumulation of radioactive iodine thus preventing thyroid cancer.

life threatening medical emergency, fever, agitation, tachycardia and tachypnea, dehydration, profuse sweating, delirium, psychosis, coma.

How do you treat thyroid storm?

A

PTU is preferred, SSKI and Lugols solution PLUS propanolol PLUS dexamethasone PLUS cooling.
