Prevention and treatment of DVT
Prevention of thromboembolism in patients with prosthetic heart valves
Prevention of stroke in patients with AF and in patients with previous MI and increased embolic risk
Dose Range
Usually start with 5 mg daily for 2 days then adjust according to INR.
Usual maintenance dose, 1–10 mg daily.
Target INR 2-3 except for heart valves which is higher.

Extra tips:
• use a non-slip bath mat when bathing
• use a soft bristle toothbrush
• use gloves when gardening and have a clean bandage with you in case you scratch yourself and begin to bleed
• use an electric shaver
Monitoring
- unexplained bruising, prolonged bleeding, dark red or brown urine, or dark red or black faeces
