A

a) Ovulatory dysfunction (e.g., hypogonadotropic hypogonadism, polycystic ovarian syndrome)
b) Tubal and peritoneal abnormalities (e.g., pelvic inflammatory disease [PID])
c) Uterine and cervical factors (e.g., fibroids)
